To prevent spam and abuse, a one-way hash of your IP address (from which the original IP cannot be recovered) is stored. The original IP address is not stored.
2. Why the information is used
- To contact you by phone and confirm your appointment
- To coordinate your serial number and chamber schedule
- To prevent misuse and spam submissions of the form
Your information is never used for advertising, marketing or any other purpose, and it is never sold or transferred to any third party.
3. Who can see the information
Submitted information can only be seen by the chamber’s authorised staff and the website administrator. The administrator may be notified by email when a new request arrives.
4. Cookies and tracking
For ordinary visitors, this website does not use any tracking cookies, analytics, or advertising tools. Session cookies required for security are created only when the website administrator logs in.
5. Third-party services
- Google Fonts: A connection is made to Google’s servers when the page loads, to load the Bengali font.
- YouTube: The privacy-friendly (youtube-nocookie) player only loads when you play a video. No information is sent to YouTube if you do not play a video.
- WhatsApp and Google Maps: Clicking these buttons takes you under that service’s own policies.
6. How long information is kept
Information is kept only for as long as needed to coordinate your appointment. After that, it is deleted.
7. Security
The website has form verification, spam prevention, and connection and login security measures in place. However, no method on the internet is 100% secure; we therefore request that you do not write highly personal or sensitive medical information in the form, and instead share it directly at the chamber.
8. Your rights
You may request to view, correct, or delete your submitted information at any time.
